How do I get a copy of my medical records?
The fastest and easiest way to obtain a copy of your medical records is to fill out a Records Release form that is HIPAA compliant. This can be found if you click here. When completing the form, please print clearly and include a complete mailing address. If the patient is a minor or is not competent to give consent, the signature of a parent, guardian or other legal representative will be required. The completed form can be faxed to 914-422-8248. While faxing the form is the fastest and easiest way to obtain your records, if you are unable to fax the form you may mail it to us at:

Do I have to pay for my medical records?
If you are requesting copies of your medical records for yourself, yes, it is $1.00 per page based on NYS Public Health Law 18.
What if I want my records to go to another doctor?
As a courtesy, we do not charge for other physicians to receive copies of your records whether it is for a second opinion or you are transferring care. You still need to fill out an Authorization.
How long does it take to get my records?
Normal processing time takes 3-5 business days.
Can I pick up a copy of my records?
Yes, you can pick up your records during our regular business hours.
Can my records be faxed to my physician or myself?
No. We are unable to this because we want to ensure the confidentiality of your medical information.
